Our mission is to help homeless and low-income families achieve sustainable independence through compassionate care, hospitality and professional support. Teaming with faith communities, Family Promise provides warm and welcoming environments for families to call home while they get back on their feet and into sustainable housing. A centrally located Family Day Center provides showers, laundry and a place for parents to look for jobs and housing. A dedicated board of directors oversees and manages the program, providing support to its staff.

    Family Promise of Linn County is looking for a Care Coordination Manager Salary: $50,000 - $60,000 range The Care Coordination Manager will be responsible for overseeing a comprehensive support program for families seeking asylum in the United States. He/She will coordinate resources for the physical shelter services that Family Promise of Linn County (FPLC) will be providing. Initiatives:1) financial support for homelessness prevention and diversion in conjunction with Waypoint Services and 2) volunteer engagement to support employment life skills development in conjunction with the Opportunity Center mini-job fairs at Ladd Library. Additionally, he/she will train and coordinate volunteers and coordinate Board needs. Some fund development and public relations activities are anticipated. He/She should exhibit excellent communication skills, both written and verbal. Key Responsibilities Direct Client Care and Coordination ● Asylum Seekers Resettlement Coordination: ○ Assess family needs and develop family plans and goals upon intake to the program. ○ Coordinate community resources across multiple local agencies to provide services including health care, food, clothing, legal and education needs for families seeking asylum in the US. ○ Monitor, track, and record all client progress reports ○ Monitor progress of families after transition from our shelter to provide ongoing support as needed ○ Maintain client confidentiality. FPLC Care Coordination Manager ● Prevention and Diversion Program: ○ Interface with Waypoint on all client referrals for utility support, landlord mediation, and temporary stays for our families ○ Coordinate with the Treasurer. ● Life Skills Support: ● Other duties as assigned, to include ○ Recruit, train and coordinate direct care volunteers ● Provide communication and coordination between community partners ● Support Board goals and objectives laid out in FPLC Strategic Plan ● Liaise with board committees ● Serve on local coalitions and attend interagency meetings ● Build good relationships with agency partners ● Report monthly to the board ● Answer phone calls and direct to appropriate sources ● Build social media presence in our community with committee help. ● Oversee newsletter content ● Support Board in coordinating fundraising/public relations in our community. ● Possible public speaking ● Strong attitude of compassion and sensitivity towards others of diverse backgrounds ● Understanding of trauma informed care principles ● Ability at problem solving ● Effective verbal communication skills ● Effective written communication skills ● Highly organized with a strong ability to multitask ● Knowledge of our community’s nonprofit world ● Experience HMIS helpful Education: BA in Social Work, or 3 years comparable work experience.
